Can you develop uremia certainly if proteinuria does not turn negative?

Not certainly, in terms of protecting kidney function and preventing uremia, the lower the proteinuria the better. However, proteinuria is only one of the indicators affecting renal function progress, but to what extent can proteinuria be treated will be affected by such aspects as pathological type, treatment time and complications.

For these three types of kidney diseases, proteinuria is difficult to completely turn negative and eliminate, but the prognosis is good, and the probability of uremia is very low:

Chronic nephritic syndrome

Chronic nephritis is one of the mild pathological types of nephropathy. In the early stage, occult blood, edema and proteinuria will occur. It is difficult to make proteinuria turn negative, but if you can stabilize it at 0.3-0.5g, the progression will be slowed down, and it can hardly reach uremia.

IgA Nephropathy class 2-3

IgA Nephropathy is a primary glomerular disease. In early stage, it is accompanied by hematuria and varying degrees of proteinuria. According to Lee's classification, pathological damage degree of IgA Nephropathy can be divided into five classes. With class 1, most glomeruli are normal; kidney damage is slight; proteinuria and hematuria can turn negative with active treatment. With class Ⅱ or Ⅲ, there will be focal segmental damage in the kidneys; few glomeruli are damaged; and the cells show mild fibrosis. Active treatment can control the progression of kidney disease. Long-term treatment can help your disease be stabilize at current stage, but sometimes proteinuria and blood urine still exist. And the risk of kidney failure is very low. If progressing to Ⅳ, Ⅴ, more than 50% of glomeruli have sclerosis, crescent of glomeruli is more than 45%, renal damage is serious, and it is not easy to control.

Membranous Nephropathy Ⅰ - Ⅱ stage

Membranous nephropathy is a typical pathological type of nephrotic syndrome. Large amounts of proteinuria and hypoalbuminemia are the main symptoms. The pathological type is divided into 4 stages. The first two stages are mild. When proteinuria is brought under 0.5g or even lowered, the prognosis will be better. Usually, with hormone + cyclophosphamide or cyclosporin treatment, some patients can get remission.

This shows that early detection and early treatment of all kidney diseases is more conducive to the treatment of the disease. In addition, although proteinuria is a measure of renal function, it is not the only indicator, and even though it can not turn negative, you won’t progress into uremia. For most patients, as long as proteinuria is under 1g, their illness condition is stable, and won’t go into uremia.
